Course Title: Advanced Skin Care Services NC III (Hybrid Delivery)

Training Provider: MYA International Training Center Inc.

1. Enrollment and Eligibility
  • Pre-requisites: Applicants must meet the entry requirements set by TESDA, which may include a National Certificate (NC) II in Beauty Care or equivalent industry experience.

  • Documentation: Students must submit valid identification, previous certifications, and medical clearances as required by the institution.

  • Verification: Enrollment is only confirmed upon verification of documents and payment of the required reservation fee or tuition.

2. Hybrid Delivery Mode

This course is delivered through a Blended Learning approach:

  • Online Component: Theoretical modules, video lectures, and written assessments delivered via Zoom / Google Meet / Google Classroom. Students must have a stable internet connection and compatible devices.

  • Offline (Face-to-Face) Component: Hands-on practical training, clinical demonstrations, and institutional assessments held at the training center.

  • Schedule: The training center reserves the right to adjust schedules due to force majeure (e.g., typhoons, government-mandated lockdowns).

3. Fees and Refund Policy

In accordance with TESDA Circular No. 029, s. 2019, the following refund policy applies:

  • 100% Refund: If the student withdraws 10 days before the start of the first training session.

  • Pro-rated Refund: If the student withdraws within the first 25% of the total course duration (based on training hours).

  • No Refund: No refunds will be issued once more than 25% of the course duration has passed, except for highly justifiable reasons (e.g., serious medical emergency) subject to management approval.

  • Non-Refundable Fees: Reservation fees and registration fees are generally non-refundable but may be transferable to another intake at the school’s discretion.

4. Attendance and Conduct
  • Attendance: Students must maintain a minimum of 80% attendance for both online and offline sessions to qualify for the Institutional Assessment.

  • Online Etiquette: Professional behavior is expected in virtual classrooms. Recording of sessions without permission is strictly prohibited.

  • Physical Safety: During offline sessions, students must wear the prescribed Personal Protective Equipment (PPE) and follow Occupational Health and Safety (OHS) protocols.

5. Health, Safety, and Liability Waiver

Because Level 3 Aesthetic Services involve advanced procedures (e.g., chemical peels, micro-needling, or light therapy):

  • Assumption of Risk: The student acknowledges that practical training involves certain risks. The student agrees to follow instructor guidance strictly to prevent injury to themselves or their models.

  • Waiver: To the extent permitted by Philippine law, the Training Provider shall not be held liable for injuries resulting from the student’s negligence or failure to follow safety protocols.

  • Medical Disclosure: Students must disclose any allergies or medical conditions that may affect their ability to perform or receive skin treatments during practice.

6. Data Privacy (RA 10173)
  • Consent: By enrolling, the student consents to the collection and processing of personal data (names, photos, assessment results) for TESDA reporting and school records.

  • Confidentiality: The Training Provider will not share student information with third parties without written consent, except for compliance with TESDA or other government requirements.

7. Assessment and Certification
  • Institutional Assessment: Students must pass the school’s internal assessment before being endorsed for the National Assessment.

  • National Assessment: Completion of the training does not guarantee an NC III Certificate. The student must undergo and pass the National Competency Assessment conducted by a TESDA-accredited Assessment Center.

8. Governing Law

These Terms and Conditions are governed by the laws of the Republic of the Philippines. Any disputes shall be settled through mediation, or if necessary, in the courts of Pampanga.
